Application Requirements and Procedures
Applicants must submit the scholarship application, resume, letters of recommendation, personal statement essay, and scanned copies of supporting documents as indicated below in electronic pdf version as attachments by email between October 1st and May 1st. You may "black out" the first 5 numbers in your social security number when it appears on copies of documents submitted with your application for your protection. Please note, application packages will not be accepted by mail.

Application packages should include the following:
1. Completed application (download application pdf below)
2. Resume
3. Three (3) letters of recommendation including:
- At least one academic recommendation
- At least one personal recommendation
4. A personal statement essay including:
- Explain your career aspirations and your educational plan to meet these goals.
- Explain how you have helped your family or made your community a better place to live. Please provide specific examples.
- Describe a personal accomplishment and the strengths and skills you used to achieve it.
- Describe a challenge or obstacle you faced in the last ten years. What did you learn about yourself from this experience?
- Discuss how you will positively impact your community upon completion of your academic career.
5. A personal statement essay of financial need.
6. Copies of the following:
- Student Aid Report (SAR) once received, after completion of the Free Applications for Federal Student Aid (FAFSA)
- Birth Certificate
- Driver’s License, State Identification, and/or Passport
- Social Security Card or Permanent Resident Card
- High School Transcript(s)
- SAT and/or ACT scores (High School Seniors only)
- College/University Transcript(s) (if applicable)
7. Procedures specific to your college/university for awarding the scholarship if you are selected as a scholarship recipient.
8. Photo and short biography for "Student Stories" page on website or other media if selected as a finalist or scholarship recipient (Optional)
9. Contact information for us to provide information about the scholarship to your high school so that your scholarship can be announced at an awards or graduation ceremony if you are selected as a scholarship recipient (Optional for high school students)
